I. ISSUANCE OF IDENTIFICATION CARD
A. ID Validation for Enrolled Students
During enrollment a student should:
1. Present his/her enrollment form at the Accounting Office and pay the necessary fees;
2. Remove old stickers from his/her perpetual ID Card;
3. Present his/her Perpetual ID card, class schedule, official receipt and ID validation slip at the ID validation counter (5th floor library building) for processing; and
4. Get his/her perpetual ID card, official receipt and class schedule from the Library Staff after they had processed it.
B. Validation for Lost ID
Secure two (2) copies of Request for Replacement of Perpetual ID Card form from the Student Affairs Office (Diego Silang Bldg.) and fill up all the necessary information. Accomplish in duplicate;
Proceed to the 5th floor library building and secure an ID Duplication form;
Accomplish the ID Duplication form and attach one (1) 2x2 size close-up facial picture;
Attach the duly accomplished ID Duplication form to the Request for Replacement of Perpetual ID form;
Proceed to the Office of the Director of Libraries and to the Student Affairs Office for verification;
Pay the corresponding amount for the ID Validation sticker at the Accounting Office. The Accounting Office will issue an official receipt and an ID validation slip;
Get the perpetual ID card from the ID Processing Center (S315);
Present perpetual ID card, official receipt and ID validation slip at the 5th floor library building; and
Validated ID card and official receipt shall be issued.
C. For Replacement of ID Validation Sticker
1. Pay the corresponding amount for the ID Validation sticker at the Accounting Office. The Accounting Office will issue an official receipt and an ID validation slip; 2. Present perpetual ID card, official receipt and ID validation slip at the 5th floor library building; and
3. Validated ID card and official receipt shall be issued.
D. Processing of Special ID
A student/reviewee applying for special ID should:
1. Secure four (4) copies of the special ID form from his/her respective College Dean;
2. Accomplish the necessary information and forward the same to their Department Head, College Dean, Vice-President for Academic Affairs and Director of Libraries for endorsement and approval;
3. Proceed to the Accounting Office for the release of the Special ID Card; and
4. Proceed to the 5th floor library building for registration of the ID card.
II. ISSUANCE OF BORROWER'S CARD
A. Request for New Borrower's Card
1. New students and faculty securing a borrower's card for the first time are required to submit two (2) 2X2 pictures and present I.D. card for proper identification;
2. Accomplish the registration form/logbook; and
3. Check the accuracy of information indicated on the borrower's card.
Color-coding:
Green Undergraduate Students
Blue Graduate Students
Orange Teaching Personnel
Yellow Non Teaching Personnel
B. Request for Replacement of Borrower's Card
1. Lost Borrower's Card
A lost borrower's card should be reported immediately to the library staff in charge of the 5 th floor Library Building
I A fee of Php40.00 is charged for the replacement of a lost borrower's card. A borrower's card is non- transferable.
A Student should:
a. Secure three (3) copies of Borrower's Card Duplication Slip from the 5th floor library building;
b. Proceed to the Accounting Office for payment of required fee;
c. Present the borrower's card duplication slip, official receipt, and one (1) 2x2 size ID picture to the Library Personnel at the 5th floor library building; and
d. Accomplish the logbook provided for borrower's card.

9 AM to 11 AM (daily)
2 PM to 4 PM (daily)
2. Filled-up Borrower's Card
A new borrower's card can be issued free of charge as a replacement for old borrower's cards in the following circumstances:
a. Normal wear and tear;
b. No available space to record additional books borrowed for overnight; and
c. Misspelled names. ( Note : Slight typographical errors on entries made on the borrower's card can be corrected without changing the entire card).
Borrower's card showing signs of carelessness and/or intentional damages such as but not limited to spills, discolorations, burns, and cuts does not qualify for replacement.
III. CLEARANCE
A. During enrollment, a student must settle any library accountability. A clearance form must be presented prior to ID validation.
B. A faculty or non-teaching personnel who is about to retire will be given a general clearance form to be signed by the Director of Libraries to indicate that the faculty member does not have any library accountability. In addition, the library will not issue temporary clearance unless the borrower submits a letter stating that he/she will pay such liabilities before he/she resigns or retires.
C. A student who decides to withdraw from the university and/or wants to secure his/her diploma and other school records will be required to process a library clearance. |
